Health Forms
Each year new health forms are required for anyone attending any event. It is important we keep these on file in the event of any emergency. If you fill one out at the beginning of the year, we will keep it on file for the entire year. If a family member decides later they would like to attend, they can complete one at that time.
We will have blank copies available at meetings to complete; however, you can save time by completing the form on your computer and printing it out. You can save the completed copy and open it up each year to make the minor changes.
Only Parts A & B are required for Cub Scouts
